  • File a complaint if you believe your rights have been violated

Our Uses and Disclosures

We may use and disclose your protected health information (PHI) for:

  • Treatment – to provide you podiatric care (e.g., scheduling surgery for bunions, treating plantar fasciitis, or diabetic foot care)

  • Payment – to bill your insurance or collect payment

  • Health Care Operations – to run our practice, improve care, and comply with legal requirements

We may also share information for:

  • Public health and safety

  • Health oversight activities

  • Judicial and administrative proceedings

  • Law enforcement (as required by law)

  • Workers’ compensation

  • Research (with proper approvals)

  • Coroners, funeral directors, and organ donation

  • Substance use disorder records (if applicable under 42 CFR Part 2)

We will never sell your information.

  • Limit marketing communications (we rarely send these)

  • Opt out of certain fundraising communications (if any)
